At the start of 2021, MacDonald developed a cough that wouldn't go away. Joe then noticed a lump on his neck and felt he needed medical attention. Puppala examined MacDonald and diagnosed him with asthma. His records indicated he didn't see a lump on his neck. He prescribed puffer medication for MacDonald and instructed him to return in a month if his symptoms didn't improve.

The CT scan report was read by Puppala on June 7, 2021, and he wasn't concerned that it was a sentinel node . The doctor's theory was MacDonald might have a vaccine-related adenopathy, something he had seen in health-care workers. In Nov. 29, 2021, MacDonald's godfather brought him in to see Puppala again because MacDonald needed disability paperwork filled out. Since MacDonald was still not feeling better and the lump hadn't gone away, Puppala ordered a biopsy to"rule out malignancy." The CT scan results were not shown or discussed with MacDonald or his godfather during the visit. Puppala said he told the godfather he wanted MacDonald to get a biopsy.

In June 2022, Joe went to see Puppala and told him of MacDonald's diagnosis, upset over the amount of time that had passed. MacDonald's treatment started immediately under the care of a different doctor and Joe said he is currently in remission.Allan MacDonald rests at his Sydney home during the time he was undergoing treatments for stage 4 lymphoma. The committee noted that they were concerned that Puppala did not make sure he knew who MacDonald's substitute decision-maker was, even though he told them he didn't think MacDonald understood the medical information being given to him.
