B.C. government investigation determines dam collapse was caused by weak foundation, design problems with the dam, poor on-site monitoring and other factors

The matter will be heard in the Supreme Court of British Columbia where an indictment was filed on Dec. 6. In the years after the spill, an investigation by the B.C. government in conjunction with external independent engineers and consultants determined the collapse was caused by many factors, including a weak foundation, design problems with the dam, poor on-site monitoring by Imperial and its consultants and subpar management of water buildup.
The Mount Polley spill is considered one of the worst environmental disasters in the Canadian mining industry, with hundreds of tonnes of toxic materials such as arsenic, lead, and copper spilling into nearby lakes.
