ARCH Hospice is seeking a temporary full time Family Support Facilitator, from the date of hire for 6-months.
As a member of an inter-professional team, the family support facilitator provides support to residents, families and friends as part of a collaborative, inter-professional team. This position helps facilitate grief and bereavement groups, as well as education on grief and bereavement, individually and as a part of the supportive care team.
5. Comfort discussing death and dying and ability to work with resident/family living with and dying from advanced illness.7. Strong organizational and prioritization skills.9. Highly developed problem solving, critical thinking and conflict management skills.
