The StarPhoenix obtained the internal investigation report in the 2017 death of Chris Van Camp through an access to information request.
Her lawyer, Tavengwa Runyowa, said the CSC report formalized what he and his client have always believed and known from other sources.
The CSC report on his death said Van Camp was not assessed by the prison’s health staff when he was readmitted, since he arrived at 9:45 p.m. and the health department closed at 7 p.m. Van Camp and Vandewater had shared a cell for four months previously with no problems, which led correctional security intelligence officers to conclude there was no basis for concern about them once again sharing a cell. Both inmates were interviewed and assessed, but because the cell assignment was made after hours, a correctional officer relied on information in the computerized case management system and the inmates’ interviews.
It adds that investigation further found that a couple of patrols “lacked in quality,” noting one patrol lasted only 32 seconds. Surveillance footage showed one guard “did not utilize a flashlight consistently to look in all cells” and didn’t “stop at each cell to confirm the well-being of each inmate,” the report says.Article content
